Mount Zion Health is a community health service organisation. We promote clients’ health and wellness through the delivery of therapeutic services by skilled and experienced healthcare workers. We are registered with the NDIS as a service provider. Our NDIS participants are empowered to have choice and control over their own care, needs and interests.

Our Services
DISABILITY SERVICE
Daily Personal Activities
We provide support with activities of daily living like personal hygiene, grooming, and feeding.
Daily Living Life Skills
We provide support to accomplish tasks like travelling for medical appointments, shopping, banking, site seeing and community events.
Participate Community
We support with visits to social events or public places like sports centres, parks, cinema, café, clubs, and beach.
Household Tasks
We offer support with cleaning, cooking, laundry, gardening, and general housekeeping.
Assist Life Stage Transition
We provide capacity building support to clients to reduce future costs. We monitor services purchased to ensure reasonable and necessary use of allocated funds.
Community nursing care for high needs
Our trained and experienced specialist nurses deliver general nursing care to clients in the community. This include but not limited to medication administration and other procedures approved by the Australian Health Practitioner Regulation Agency (Aphra) for registered nurses.
Accommodation and Tenancy Assistance
We provide support independent living (SIL) and respite accommodation.
Assistance with Travel/Transport
Specialist Positive Behaviour Support
We work with children and adults presenting with behaviour of concerns to:
- Identify the causes and purpose of the behaviour.
- Develop strategies that promote positive behaviour.
- Develop a customized care plan to regulate emotion and develop adaptive skills.
- Provide capacity building support to carers and family to promote client’s dignity, and safety of all stakeholders.
Psychosocial recovery coaching
We work with participants with mental health disorders such as depression, schizophrenia, bipolar affective disorder, anxiety and depression to implement their recovery plans. We collaborate with participants, carers and service providers to build functional capacity to:
- Manage activities of self-care.
- Attend community events.
- Manage housing/accommodation.
- Organise and attend appointments with services.
- Maintain social connection with others.
- Engage in skill training or education.
- Access other services like AOD, Men behaviour change program..
